Hospital at Home

The Children's Hospital at Home service for Oxfordshire is delivered by Oxford Health NHS Foundation Trust in conjunction with Oxford University Hospitals NHS Foundation Trust.

The Children’s Community Nursing (CCN) Hospital at Home service operates 8am-8pm, 7 days a week. The service provides clinical assessments and management of acutely unwell children in the community in the Oxfordshire area. The service provides telephone, virtual and/or home assessments following specific referral criteria and care assessment pathways. The service works in partnership with the Acute Paediatric Team at Oxford University Hospital NHS Foundation Trust to safely manage children in the home setting and to escalate any concerns regarding a deteriorating child.

GP's based in Oxfordshire within Oxfordshire ICB can refer patients to the Children's Community Nursing Hospital at Home Team for Bronchiolitis or Gastroenteritis. Wheeze patients can be considered following discussion with the on call Paediatric advice line.

The referral form is on EMIS and must be emailed to CCNTeam@oxfordhealth.nhs.uk
